Customer Appointment Registration

Beethoven Pianos COVID 19 Protocol:
-Every employee and customer must wear a mask at all times on our premises.
-Employees will have their temperature screened at the start of every shift.
-Customers must fill out a registration form before entering the store.
-Surfaces and pianos are regularly sanitized.
-Please maintain a social distance of 6 feet.

By submitting the registration form below, you are acknowledging that an inherent risk of exposure to COVID-19 exists in any enclosed place where people are present. By entering our store, you and any guest voluntarily assume all risks related to exposure to COVID-19 and agree not to hold Beethoven Pianos; or any of its affiliates, directors, officers, contractors, or employees liable for any illness or injury.
